I tend to forget concepts and formulae while giving the mock, and this has been a consistent phenomenon. Any advice on how to come out of it?

Try to revise as much as you can as only revision will help you to remember the formulas . Stress more on important topics more which include Arithmetic , Algebra and Geometry .
