Hello, this is regarding MDI form...the form has 5 subjects for 12th std. so which subjects to fill in the form?

For Date of Birth proof you can upload your mark sheet only if it contains your date of birth and if not then you can upload the photocopy of your Transfer Certificate .
For subjects, as gokul said, you can leave your other language if any .

You can leave out the Second Language you had in 12th.
