Let the number of questions got wrong be x.

The number of questions got right will be = 94 - x

For every right question he gets 4 marks and for every wrong question 1 mark is subtracted.

Marks for right answers = 4(94 - x) = 376 - 4x

Marks subtracted for wrong answers = x

376 - 4x - x = 141

376 - 141 = 4x + x

235 = 5x

x = 235/5

x = 47

x represents the questions wrongly answered.

The number of questions wrongly answered is thus 47.
